To schedule an appointment, text (479) 304-9431 and ask for our office manager. She will then give you the information about Sholom and the
Sholom Portal, where paperwork can be completed.

We will need your name, birthday, address, phone number, and email to open a chart
and schedule. A secure portal will then be sent to your email.

Location

The office is located at 745 E. Joyce Blvd., Suite 201, Fayetteville, AR 72703, above Chico’s. There is a staircase as well as an elevator to access the second level. Further directions or questions can be obtained at the
